Job Description

The Access Coordinator II tasks will include patient intake and
scheduling, reporting data from HER or other sources, and providing
onsite support. They will support the Project Specialist in gathering
data for invoicing as instructed. He/she will assist a team of
physicians, APRN's, mental health professionals, and other clinical
provides to aid in the coordination of care throughout the youth's
commitment.

Responsibilties:
1.Develops procedure for managing intake by gathering patient health
information and insuring completion of required forms
2.Obtains medical record provider information/forms
3.Contacts DYS and residential provider staff as appropriate
4.Inputs referral information into HER/databases within 2 days of
receipt and with accuracy
5.Updates information as required
6.Assists clinical staff in gathering data and contacting outside
agencies for information as requested


Minimum Qualifications:
High School diploma/GED plus 2 years administrative experience in health
care environment. Must have Microsoft Office and computer/basic keyboard
skills and general knowledge of office equipment (printers, scanners,
etc.) Knowledge of medical terminology preferred
